This form displays the taxes you paid to Puerto Rico from a stock that you hold or have held in your account(s). This information is also included in your 1099 issued by Osaic Inc. If you have a tax preparer, they can help you with this form, or if you're filing taxes on your own, you can enter this information on IRS form 1116.
